What is Support at Home?

Support at Home is the new Australian Government aged care program replacing the Home Care Package (HCP). It provides funded support so older Australians can stay independent in their own homes for longer.

    Day 1

    Top1Care assists you to register with My Aged Care and submit the assessment request.

    Information check

    My Aged Care calls to verify your details and refers you to an assessment provider.

    Home assessment

    4–6 weeks

    The assessment provider books a time and conducts the in-home assessment.

    Result

    Within 2 weeks

    Result becomes available online and a letter is mailed to your home.

    Wait for funding

    7–10 months

    My Aged Care releases funding through the National Priority System.

    Once activation code arrives

    Once your activation code arrives and the service agreement is signed, Top 1 Care designs a personalised Care Plan around your needs, then our professional team begins service rostering.

Assistive Technology & Home Modifications

Assistive technology and home modifications are products, equipment, and changes to your house that help you to live at home or within your community. This support is available through the Assistive Technology and Home Modifications (AT-HM) scheme, which is part of the Support at Home program.

Assistive Technology (AT)

Products and equipment that make daily tasks easier, safer and let you keep doing activities independently.

  • Communication aids — alternative & augmentative communication products

Changes to your home that make it safer and more accessible, so you can live safely at home for longer.
